After securing an internship, and an initial meeting with your on-site supervisor, fill out Form A to apply to register for the Internships course. Your on-site supervisor will also need to fill out and sign Form B.

After completing your internship, your on-site supervisor will need to fill out an evaluation form, Form C, before your final submission deadline ie one week after the last day of classes for that term or semester.

After completing your internship, please fill out Form D, which is your own anonymous evaluation form to help future students looking for internships at the same company or in a similar company, field or position.
